Process for admission to The Immaculate Conception School


Parent must complete the school application form and provide all required documents listed below. Following an evaluation of the materials, along with an interview of Parent and school visit by the child, the Parent will be notified about the status of the child.

SUBMIT THE FOLLOWING ITEMS:


1. Complete both sides of application w/signature

2. $50.00 non-refundable application fee per child

3. Birth Certificate

4. Baptismal and or Communion Certificate (if applicable)

5. Immunization Records

6. Report Cards (3 years)

7. State Test and/or other Standardized Tests (3 years)

8. Textbook Loan Form (included in packet)

Originals are needed for verification
